Dal Pozzo was the Archbishop of Pisa and a special adviser to Ferdinando de' Medici, Grand Duke of Tuscany from 1587. Bernini was commissioned to carve the bust by the sitter's nephew and protégé, Cassiano dal Pozzo, a celebrated collector, antiquarian and patron (notably of Nicolas Poussin). The powerful characterisation is remarkable in a posthumous portrait, and the sculptor must have relied on the likeness on painted portraits and perhaps a death-mask.
